Boeing 767-300ER accident report
Sunday 26 May 1991 Lauda Air Boeing 767-3Z9ER OE-LAV c/n 24628
ISP Flight NG004 from Bangkok (BKK) to Vienna (VIE) with 10 crew and 213 passengers. Uncommanded in-flight deployment of the left engine thrust reverser, short after take-off. As a result the aircraft stalled and control was lost. The aircraft entered a high speed dive, which the crew could not stop and the aircraft broke up in flight. First flight : 1989. ISP Flight MS990 from New York (JFK) to Cairo (CAI) with 15 crew and 202 passengers. Cause : first officer shut the engines off in flight pushed the nose of the aircraft down in a steep dive, from which there was no recovery. The reason for the actions of the first officer could not be determined. First flight : 1989. Airframe hrs : 33219 cycles : 7556 engines : PW4060. All people on board died in the crash.
